Why in news
India is planning to introduce a dengue vaccine by next year. The first nationwide mapping of dengue virus strains reveals all four serotypes circulating simultaneously in eight states, which could influence vaccine strategy. Concurrent infections with multiple strains are linked to more severe disease, making this surveillance critical for immunization policy.
Background
Dengue is a mosquito-borne viral disease with four distinct serotypes (DENV-1 to DENV-4). India has been hyperendemic for dengue, but comprehensive molecular surveillance was lacking. Several dengue vaccine candidates are in advanced stages of development, and vaccine efficacy varies against different serotypes.

ICMR-National Institute of Virology, Pune led the pan-India molecular surveillance study.
The study analyzed 6,889 lab-confirmed dengue samples from 45 VRDLs across 25 states/UTs between 2023 and 2025.
All four dengue serotypes (DENV-1, DENV-2, DENV-3, DENV-4) were detected simultaneously in Andhra Pradesh, Gujarat, Haryana, Madhya Pradesh, Maharashtra, Rajasthan, Tamil Nadu, and West Bengal.
DENV-2 was the dominant strain, accounting for 51% of infections; followed by DENV-3 (13%), DENV-1 (7%), and DENV-4 (1%).
About 7% of patients had concurrent infections with two or more serotypes.
Concurrent infections were associated with higher risk of thrombocytopenia, hemorrhagic manifestations, and arthralgia.
Regional variations: DENV-1 was more common in Tamil Nadu (29%), Haryana (38%), and Rajasthan (25%); DENV-3 was more frequent in Gujarat (30%), Tamil Nadu (29%), Uttar Pradesh (29%), and West Bengal (20%).
Continuous molecular surveillance is essential for outbreak prediction and vaccine strategy.
